Our Story

Rise and Thrive Care was founded with a simple but powerful purpose to bring heart, humanity, and hope back into care.

Co-founded by two dedicated professionals with extensive experience across the disability, nursing, and community sectors, Rise and Thrive Care was born from a shared belief that every person deserves to live with dignity, belonging, and empowerment.

Through years of working in frontline and leadership roles, they witnessed moments where people were cared for but not always connected with. They saw families seeking reassurance, participants longing for independence, and staff yearning to feel inspired by the difference they make.

Rise and Thrive Care was created to change that. To build a service that feels personal, purposeful, and deeply human. A place where every individual Feels supported, Seen and celebrated, families are included, and staff are valued for the compassion they bring each day and the small changes they make. 

Our approach is guided by our core belief that true care begins with understanding with taking the time to listen, to nurture, and to walk beside each person as they rise beyond challenges and thrive in their own unique way.
